We recognize family as the cornerstone of our company, and want you to be part of a team that values your contributions and well-being.<br /><br />We value our families and community:<br /><br />* Flexible Schedule<br />* Hardship Program- program to support staff during challenging times<br />* Patriot Cares- nonprofit for community outreach<br /><br />How we support you professionally:<br /><br />* This position is paid at a competitive hourly rate<br />* Mileage reimbursement<br />* Company 401K with 4% company matching<br /><br />The responsibilities of our Occupational Therapists include:<br /><br />* Assist the physician in evaluating the patient's level of function by applying diagnostic and prognostic procedures<br />* Conduct initial and ongoing comprehensive assessments of the patient's needs, including Outcome and Assessment Information Set (OASIS) assessments<br />* Assure that the physician's orders are appropriate and discusses necessary changes<br />* Teach, supervise, and counsel the family and patient in the total Occupational Therapy program and other related problems of the patient at home<br />* Communicate with the physician on a regular basis regarding patient's status, obtaining additional orders, if necessary<br /><br />Qualifications:<br /><br />* Must be a graduate of an Occupational Therapy school approved by the Council of Medical Education of the A.M.A. in collaboration with the American Occupational Therapy Association<br />* Preferably should be a member of the American Occupational Therapy Association<br />* Must be licensed by the state<br />* Must have at least one year of institutional experience<br />* Must have a criminal background check<br />* Must have current CPR (BLS) certification<br /><br />Job Conditions:<br /><br />* The ability to drive and ability to access patients' homes which may not be routinely wheelchair accessible are required<br />* Hearing, eyesight, and physical dexterity must be sufficient to perform a physical assessment of the patient's condition and to perform patient care<br />* On occasion, may be required to bend, stoop, reach and move patient weight up to 250 pounds; lift and/or carry up to 30 pounds<br /><br />Patriot at Home is an equal opportunity employer<br /><br />Job Types: Full-time, Part-time, PRN<br /><br />Benefits:<br />* 401(k)<br />* 401(k) matching<br />* Flexible schedule<br />* Mileage reimbursement<br /><br />Work Location: On the road
