*JOB SUMMARY: *<br /><br />The *occupational therapist* employed by the Agency is responsible for the implementation of standardsof care for occupational therapy services and for the adherence to all conditions outlined in their scope of practice.<br /><br />*JOB QUALIFICATIONS:*<br /><br />To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required. Graduate of an occupational therapy curriculum accredited jointly by the Committee on Allied Health Education and Accreditation of the American Medical Association and the American Occupational Therapy Association.<br /><br />2. Registered with the National Registration of the American Occupational Therapy Association.<br /><br />3. Two years of clinical experience as an occupational therapist.<br /><br />4. Community/home health experience is preferred.<br /><br />5. Possesses and maintains current CPR and BLS certification.<br /><br />6. Must have valid Florida driver's license, a reliable automobile and minimum insurance as required by law.<br /><br />7. Licensed to practice as occupational therapist within the State.<br /><br />*JOB SUMMARY: *<br /><br />The *physical therapist* contracted or employed through the Agency is responsible for providing physical therapy services to promote positive therapeutic outcomes and for adherence to all conditions outlined in the Personnel Services Agreement.<br /><br />*JOB QUALIFICATIONS:*<br /><br />To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.<br /><br />1. Graduate of a physical therapy curriculum accredited jointly by the Committee on Allied Health Education and Accreditation of the American Medical Association and the American Physical Therapy Association.<br /><br />2. Possesses a degree from a baccalaureate, master’s, or doctorate program in physical therapy approved by an accredited organization.<br /><br />3. Minimum of two years of appropriate experience as a physical therapist.<br /><br />4. Community/home health experience is preferred.<br /><br />5. Possesses and maintains current CPR certification and BLS Certification.<br /><br />6. Must have valid Florida Driver's license, a reliable automobile and minimum insurance as required by law.<br /><br />7. Licensed to practice as physical therapist within the State.<br /><br />8.
