We are currently seeking an experienced Placement Coordinator/Staffer.

This position requires an individual who is passionate about providing quality care to our clients within New York City’s senior sector. You will work with a team of Patient Care Coordinators and assist in sourcing the most appropriate caregivers for each of the available clients. Job Description:

Educate prospective members and their families on requirements to establish home care services.

Serve as a liaison between all parties; i.e., communicate between members, nursing facilities, physician offices, hospitals, and insurance companies.

Assist with assessment scheduling & the enrollment process to ensure members are updated throughout.

Process daily member referrals received by the Marketing Department.

Updating daily reports using the Intake Bolt app.

Requirements:

Must have at least 1-year experience as a Patient care coordinator or Staffer.

Bilingual in Spanish is a plus

Self-motivated and driven – you must be a passionate individual who is unafraid to network and see an issue and find a solution

Compassionate and an avid listener – members must be heard regarding their concerns and needs as well as caregivers

A problem solver.

Professionalism in demeanor and attire is a must.

Time management skills/Detail oriented.

You will be joining a well structured team, all work gets done from 9am-5pm.

The position includes the following tasks:

Outreach, scheduling and engagement with patients

Work effectively with all departments to ensure the best delivery of service

Effectively communicate with clients and caregivers regarding care plan and services to be provided.
