Employee will be performing member outreach.
Coaching members into healthy habits and gap closures. regarding coverage and provider visits.
Assist with membership attribution
Assist with provider and patient engagement
Assist with solving for SDOH concerns
Provide resources to members to improve health outcome
Qualification:
LPN or Medical Assistant ( Nurses must have an active compact license for TN,MS, or AR)
Clinical experience working in a physician’s office
Previous clinical outbound call outreach; motivational interviewing
Case Management/Utilization Management
